Eligibility Criteria for Care Recipients:
- Individuals must be age 16 or older.
- Applicants should have a physical or developmental disability that prevents them from living safely and independently.
- Applicants must be eligible for or currently enrolled in MassHealth to qualify for certain services covered by the program.
Evaluation and Individualized Care Planning
The process begins with a detailed evaluation of the client’s condition. This assessment is conducted by our skilled team of healthcare professionals, including registered nurses and case managers. During the evaluation, we gather information about the client’s medical history, current health status, daily living activities, and any special needs they might have. This thorough understanding allows us to address the complete spectrum of the client’s health and personal care requirements.
Based on the results of the evaluation, we develop a customized care plan. This plan is meticulously tailored to address all aspects of the client’s care needs. It outlines the specific services that will be provided, the goals for the client’s health and well-being, and the roles and responsibilities of the caregiver.
The care plan includes, but is not limited to:
- Medical Management: Instructions on medication management, medical appointments, and coordination with healthcare providers.
- Daily Living Support: Guidelines for assisting with daily activities such as bathing, dressing, and meal preparation.
- Mobility Assistance: Strategies to aid with mobility and ensure safety within the home environment.
- Social Interaction and Emotional Support: Approaches to support social interaction, maintain relationships, and enhance emotional well-being.
- Emergency Procedures: Clear procedures for managing emergencies or unexpected health issues.
Once the care plan is established, the designated caregiver receives training specific to the plan’s requirements. This training ensures that the caregiver is fully prepared to implement the plan effectively. Our training sessions cover medical procedures, emergency response techniques, and day-to-day care routines, all customized to the client’s conditions and preferences.
Care needs can change over time. Therefore, our process includes regular reevaluations of the client’s condition and adjustments to the care plan as necessary. These periodic reviews ensure that the care provided continues to meet the evolving needs of the client effectively.
Throughout the entire process, our multi-disciplinary team remains actively involved, providing support, guidance, and resources to ensure that caregivers are successful and clients receive the best possible care. Caregivers also have access to 24/7 support for any immediate questions or concerns that may arise.

- Initial Inquiry: Interested candidates can initiate the application process by contacting our intake team via phone or email.
- Preliminary Assessment: We conduct a preliminary assessment to verify the eligibility of the care recipient based on their health conditions and needs.
- Documentation Submission: Candidates must submit all required documentation, including proof of age, residency, and MassHealth enrollment.
- Caregiver Training: Approved caregivers undergo a mandatory training program, which includes practical and theoretical components about care methodologies and recipient-specific needs.
- Final Approval: After successful completion of training and all assessments, caregivers are officially enrolled in our program and matched with recipients.
