What is an AFC Program?
The AFC program provides members with a comprehensive care setup that includes round-the-clock supervision to aid in managing Activities of Daily Living (ADLs). Our program ensures that each member receives the personal care, meal preparation, accompaniment to medical visits, and medication reminders they need to maintain their health and independence.
Our AFC program offers two options to ensure you receive the best possible care:
We train your loved one: If you have a trusted friend or family member who wishes to become your caregiver, we can provide them with comprehensive training to ensure they feel confident and equipped to assist you with Activities of Daily Living (ADLs) including bathing, grooming, dressing, toileting, meal preparation, medication reminders, and transportation to appointments.
We pair you with a trained caregiver: If you prefer not to have a loved one as your caregiver, we can match you with a dedicated, highly trained professional who will become an extension of your family. This caregiver will possess the skills and experience necessary to provide all the support you need with your ADLs.
This caregiver will provide essential support with Activities of Daily Living (ADLs) including:
- Bathing and grooming
- Dressing
- Toileting
- Meal preparation
- Medication reminders
- Transportation to appointments
Who Can Benefit:
Our AFC program is ideal for individuals who:
- Have a chronic medical or psychiatric condition that makes it difficult to manage daily tasks.
- Are unable to live safely on their own.
- Are 16 years of age or older.
