Job Description:
We are seeking a licensed Registered Nurse (RN) and Licensed Practical Nurse (LPN) to join our team. The nurse will play a key role in assessing clients, developing care plans, training caregivers, and ensuring compliance with AFC program requirements.
Key Responsibilities:
- Conduct initial and ongoing nursing assessments for AFC participants
- Develop and update individualized care plans
- Monitor participant health and provide clinical oversight
- Collaborate with case managers, caregivers, and other healthcare professionals
- Provide training and support to caregivers on medical tasks and care procedures
- Maintain accurate and timely documentation in accordance with regulatory requirements
Qualifications:
- Current and valid RN license or LPN in the state of Massachusetts
- Strong clinical assessment and documentation skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Valid driver’s license
